Long-Term Care Homes and Retirement Homes: COVID-19 Vaccine Information
The province of Ontario is currently focused on providing COVID-19 vaccines to the most vulnerable populations. On December 22, 2020, Sunnybrook began to provide the COVID-19 vaccine to staff in long-term care settings in the surrounding area, followed by retirement homes.
Priority for vaccination is developed in partnership with Toronto Public Health. Staff of the following homes will be vaccinated first:

Please note that staff will be contacted by their employer in the long-term care home or retirement home for booking information. The clinic is by appointment only - no walk-ins are being accommodated.
If you are a member of the general public, who is currently not employed at one of the long-term care homes or retirement homes listed above, please visit the Government of Ontario web site for more information about when the vaccine will become available to the public and the rollout and distribution plans.
Information for your appointment:
Once your appointment is confirmed, please review the directions to the clinic and where to find free parking (please see the bottom of this page). We suggest you also review information from the Ministry of Health about what you need to know before your COVID-19 vaccine appointment and general information on the vaccine. You will be screened for COVID-19 symptoms prior to entering the clinic. Please do not arrive earlier than 10 minutes before your scheduled appointment time.
Please wear a mask and bring the following to your appointment:
- Consent form – if you can, please print it in advance and sign it
- Your Ontario health (OHIP) card
- Reading glasses (if applicable)
- Loose fitting shirt or T-shirt (for easy access to upper arm)
After you receive your vaccination, you will be required wait in the observation area to monitor for symptoms for 15 minutes.
Pfizer's COVID-19 vaccine will be provided in two doses, 21 days apart. During your first appointment, we will book an appointment for your second dose. You will also receive a Proof of Vaccination receipt.

Parking information:
There is no charge for parking for those accessing the clinic. Please bring your parking receipt with you to the clinic for validation. Prior to exiting the facility, you will be directed to a self-serve parking validation kiosk.
Access instructions:
The clinic is by appointment only. The clinic can be accessed by entering through the glass doorway near Tim Horton's Breezeway on the Ground Floor (in between H-wing and E-wing). Please see map below.
